How do you want to be cared for at the end of your life? Who do you want to make decisions for you, if you can't? What kind of medical treatment do you want to receive (or not)? How comfortable do you want to be? How do you want people to treat you? What do you want your loved ones to know? We will use the Five Wishes workbook to create a power of attorney for health care reflecting your wishes, and provide resources for funeral planning. Make these important decisions now, while they are still yours to make, and make the end of life easier for you and your loved ones by planning ahead.
